For copies of surgery records, email office@purplecatvet.com.  There is a $10 fee for all record requests for appointments that occurred before November 2024.

Note: we encourage our clients to take photos of their surgery records before they leave our offices. Please check your phone for a photo of your record in order to save time and money.
